Does brachytherapy treat eye melanoma?

Suitability depends on the tumour's size and position, the condition of the eye and the person's wider health. An ocular oncology team compares plaque treatment with other local treatments or surgery before making a recommendation.

A small disc containing a radiation source is surgically secured to the outside wall of the eye, positioned next to the melanoma. Radiation travels a short distance into the tumour, reducing exposure to more distant tissue.

The plaque stays in place for a period calculated from the tumour measurements and radiation plan, then is removed in another procedure. The specialist explains precautions while it is present. The plaque is not left in the eye after the planned dose has been delivered.

Treatment can affect vision later because the retina, optic nerve, lens or other eye structures may receive some radiation. The likely visual effect varies with the tumour's location as well as the dose reaching these structures.

Give the surgical team a current medicine list and mention blood thinners, allergies, previous eye operations and any problems with anaesthesia. Follow their instructions on fasting, eye drops and medicines, and organise help with travel and tasks if your vision will be temporarily restricted.

While the plaque is fitted, observe the radiation-safety instructions for visitors and close contact. Do not rub or press the treated eye. Use prescribed drops exactly as directed and attend plaque removal and follow-up imaging appointments.

Call the eye unit urgently for sudden loss or marked worsening of vision, severe eye pain, increasing redness or swelling, pus-like discharge, fever, persistent bleeding or a protective eye shield that has shifted unexpectedly. Follow the unit's emergency route outside clinic hours.

New flashes, a shower of floaters or a dark curtain across vision also warrant urgent eye assessment. Do not wait for routine follow-up when vision changes abruptly.

Ask where the melanoma sits in relation to the optic nerve and macula, what vision may remain after treatment, which radiation precautions apply and how the tumour response will be monitored. Also clarify whether you need ongoing checks of the other eye and general health.
