Does eyelid surgery treat tear trough hollows?

A tear trough is a groove between the lower eyelid and upper cheek that can cast a shadow. Lower blepharoplasty may reposition lower-eyelid fat to blend a transition between the lid and cheek, which can make a hollow look less noticeable when fullness and a groove occur together. A true volume deficit, thin skin, pigment, or prominent cheek anatomy may remain. The procedure can also be unsuitable when the lower eyelid is lax or the eye surface is vulnerable.

Have the lower eyelid and midface assessed in good lighting, with attention to skin quality, fat position, eyelid support, cheek contour, and eye comfort. Ask whether fat repositioning, another surgical design, or a non-surgical option better matches the cause of the shadow. Discuss dry-eye symptoms, previous filler or surgery, healing time, and how subtle or visible a change is realistic.

Sudden swelling, severe pain, new double vision, reduced vision, marked redness, or a new one-sided change should be assessed promptly. After treatment, urgent contact is needed for vision change, escalating pain, significant bleeding, or rapidly worsening swelling.

Ask whether the shadow comes from a groove, puffiness, pigment, or volume loss; whether your eyelid support is strong enough; and which parts of the appearance surgery is unlikely to change.
