What the swelling can mean
After a fracture, damaged blood vessels and irritated soft tissues can make the ankle enlarge and feel tight. Bruising, tenderness, stiffness, and pain with movement may occur at the same time. Swelling can extend into the foot and may change during the day as the leg is positioned. The amount of swelling does not reliably show how severe the fracture is, and an ankle can be fractured without an obvious change in shape. An examination and imaging are needed to identify the injury.
What to do now
Avoid walking on the injured ankle until a clinician advises that it is safe. Keep it supported in the position found, use crutches if they have been provided, and elevate the leg while resting. A wrapped cold pack may ease discomfort for short periods; keep a cloth between the pack and skin. Do not massage the area or try to straighten it. Arrange prompt medical assessment after a significant twist, fall, blow, or persistent inability to use the ankle.
When to seek urgent care
Seek emergency help if the foot becomes cold, pale, blue, numb, or increasingly difficult to move, or if pain and swelling rapidly worsen. An open wound near the injury, a visibly displaced ankle, or bone showing through the skin also needs immediate care. These signs can indicate damage requiring urgent treatment. Even without them, new swelling after an injury should be assessed if you cannot walk normally or pain does not settle.
