Does anaemia cause shortness of breath?

When oxygen delivery is reduced, the body may compensate by making you breathe faster or more deeply. Breathlessness may first appear when walking, climbing stairs, or exercising and may occur with tiredness, pale skin, headaches, or a racing heartbeat. The pattern depends on the type and severity of anaemia and how quickly it developed.

Breathlessness can also result from asthma, infection, heart disease, lung disease, anxiety, or a clot. The symptom should therefore be assessed in context rather than attributed to anaemia without examination.

Book a clinical assessment for new or worsening breathlessness. Describe whether it occurs at rest or with activity, whether it wakes you at night, and whether you have cough, fever, chest discomfort, swelling, bleeding, or recent illness. A clinician may check oxygen levels, heart and lung sounds, and blood tests. Do not self-treat with iron unless advised.

Seek emergency help for sudden or severe breathlessness, blue or grey lips, chest pain, fainting, confusion, coughing blood, or difficulty speaking in full sentences. Obtain prompt medical advice when breathlessness is rapidly worsening, occurs at rest, or follows heavy bleeding.

Ask whether a full blood count is needed, what could be causing anaemia, and whether your breathing pattern suggests another problem. Ask how to pace activity safely, which warning signs matter, and when you should receive follow-up.
