Call the practice and explain why you cannot attend, where the patient is located, the symptoms, how long they have been present, and any relevant medical conditions or medicines. Keep the patient’s phone available in case the clinician needs clarification. Make the room accessible, gather medication packets and recent medical information, and have someone present if the patient needs help communicating. If a home visit is unavailable, ask whether a clinic appointment, video consultation, or another local service is more suitable.

Seek urgent emergency help for severe breathing difficulty, chest pressure, sudden weakness on one side, new confusion, loss of consciousness, heavy bleeding, or a serious injury. Do not wait for a routine home appointment when symptoms are rapidly worsening. Tell emergency services the exact location and describe what changed. A home visit is intended for assessment of a stable problem or a patient who cannot reasonably travel; it is not a substitute for emergency response.
