Does a family medicine perform chronic disease review?

A review looks at how the condition has been changing and whether treatment is helping. The clinician may discuss symptoms, home readings, medicine use, side effects, diet, activity, sleep, mood, and practical barriers. They may examine you, review test results, update prescriptions, and agree on monitoring or referrals. Conditions commonly managed in family medicine include diabetes, asthma, high blood pressure, thyroid problems, and other long-term illnesses.

Make an appointment before medicines run out and bring every prescribed or non-prescribed medicine. Take home readings or a symptom diary if you use them, along with recent reports and specialist letters. Write down side effects, missed doses, new symptoms, and goals such as walking more comfortably or sleeping better. Ask what to do if your readings change between visits.

Do not wait for a routine review for severe chest pain, major breathing difficulty, sudden confusion, fainting, new one-sided weakness, or a severe allergic reaction. Seek prompt advice when symptoms are rapidly worsening, medicines cause a concerning reaction, or home readings are repeatedly far outside the range your clinician gave you. Keep taking medicines as prescribed unless a clinician tells you otherwise.
