Do you need ICSI after semen analysis?

A semen analysis examines features such as sperm concentration, movement and shape, along with the sample volume. Results can vary between samples, and one report may not explain the whole fertility picture. Your clinician may recommend repeating the test or checking for contributing health factors before making a treatment decision.

ICSI is a laboratory fertilisation method in which one sperm is placed into an egg during an IVF cycle. It may be considered when sperm numbers or movement are substantially reduced, when sperm must be retrieved, or when previous fertilisation did not occur as expected. It is not a routine consequence of every semen result.

Request an explanation of each result and whether it is considered reliable. Discuss the options of trying to conceive without assisted treatment, insemination or IVF, depending on both partners' assessment. Ask whether a repeat analysis, medical review or fertility specialist referral is appropriate before choosing ICSI.

Arrange medical review for blood in semen, testicular pain or swelling, a sudden change in ejaculation, or repeated abnormal results. Seek urgent care for sudden severe testicular pain, because rapid assessment can protect the testicle.
