Do you need custom orthotics after biomechanical assessment?

Short answer

You may need custom orthotics after a biomechanical assessment if specific movement findings match your symptoms and footwear needs.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the assessment means

A biomechanical assessment looks at how your feet, ankles, knees, hips and trunk move during standing or activity. The clinician may review joint motion, muscle control, pressure distribution and the way your shoes are worn. These findings help explain whether an insert could alter load or support a movement problem. They do not, by themselves, prescribe an orthotic. Some people benefit from exercises, footwear changes, activity adjustment or a ready-made insert instead.

What to do next

Ask which finding the proposed orthotic is intended to address and what improvement should be monitored. Bring the shoes you use most often, describe when symptoms appear, and discuss whether a trial with a simple insert is reasonable. If a custom device is recommended, check how it will be fitted, how to introduce it gradually, and when it should be reviewed. Stop using it and contact the clinician if it causes increasing pain, rubbing, numbness or skin injury.

When to seek care promptly

Seek prompt medical assessment for a newly cold or pale foot, sudden weakness, loss of sensation, severe swelling, an open wound, or pain that rapidly worsens. These signs need assessment beyond orthotic planning. A persistent limp, repeated falls or pain that limits normal activity also merits review so the underlying problem is addressed rather than masked by an insert.

Questions for your doctor

Ask what the assessment showed, what alternative treatments fit those findings, how the orthotic should feel during use, and what would mean it is not suitable. Ask when to reassess your walking, symptoms and footwear.

Questions people ask

Does every biomechanical assessment lead to custom orthotics?

No. The assessment informs a plan; orthotics are considered only when their intended support matches the person’s findings and goals.

How should new orthotics be introduced?

Follow the fitting clinician’s schedule, increase wear as advised, and report pain, rubbing, numbness or skin changes.
