Do you need a referral for stoma surgery?

Your oncologist, gastroenterologist or another clinician managing the underlying condition may initiate it. Emergency stoma surgery follows a different pathway and does not depend on arranging a routine outpatient referral first.

A stoma brings part of the bowel through an opening in the abdomen so waste can leave the body. It may be an ileostomy or colostomy, and it may be intended for a limited period or remain in place, depending on the disease and operation.

Referral allows the surgeon to review the reason for surgery, bowel anatomy, scans, previous operations and overall health. A stoma care nurse may discuss pouching, skin care and daily life, then help identify a practical site on the abdomen before the operation.

Ask which surgeon or service will receive the request and whether a stoma nurse visit will be included. Bring scan and endoscopy reports if available, details of earlier abdominal surgery, your medicines and information about allergies or other health conditions.

Write down practical concerns about clothing, work, bathing, diet, travel, prayer, intimacy and obtaining supplies. These topics help the team tailor teaching and plan stoma placement. Follow any bowel preparation, fasting or medicine instructions only when the surgical team provides them for your procedure.

Seek urgent medical assessment for severe or increasing abdominal pain, a swollen abdomen with vomiting, inability to pass stool or gas, collapse, heavy bleeding or signs of severe dehydration. These problems should not wait for a routine surgical appointment.

If you already have a stoma, urgent review is also appropriate when it stops producing output alongside pain or vomiting, turns dark or dusky, bleeds heavily or retracts suddenly. Contact the stoma team for persistent leakage or worsening skin damage even when emergency care is not needed.

Ask why a stoma is recommended, what type is planned, whether it is expected to be temporary and what would determine possible reversal. Clarify how the operation fits with cancer treatment, how pain will be managed, when stoma teaching begins and whom to call after discharge.
