Distal Phalanx Thumb Pain: What to Know

The distal phalanx is the thumb bone beyond the last joint, beneath the nail and fingertip pad. A crush injury, cut, puncture, fracture, nail-bed injury, infection, or irritation around the thumb tendons can cause pain there. Note whether pain began after trauma, whether the nail changed colour, and whether swelling, numbness, drainage, or trouble bending the thumb is present. Pain near the thumb side of the wrist or base can reflect a different problem, such as De Quervain's tenosynovitis, rather than a fingertip injury.

Protect the thumb from further impact and remove rings or tight items before swelling increases. Keep cuts clean and covered, use a cool pack wrapped in cloth for short periods, and raise the hand. Do not drain blood beneath the nail or try to straighten a deformed thumb yourself. A clinician may need to check circulation, sensation, nail-bed damage, and bone alignment, with imaging when a fracture is suspected.

Seek urgent care after a crush or deep cut if the thumb is crooked, open, numb, pale, blue, cold, or difficult to move. Prompt review is also needed for worsening redness, warmth, swelling, pus, fever, severe throbbing beneath the nail, or pain that is not settling. A bite or puncture wound should be assessed even when the opening looks small.
