Diarrhea with fever: what to know

An infection affecting the bowel can cause loose stools, fever, cramps and tiredness. The fever may come from an illness elsewhere too: COVID-19, chickenpox and some respiratory infections can occasionally include digestive symptoms, while cellulitis or another bacterial illness can produce fever alongside unrelated diarrhea. The cause depends on stool changes, pain, exposure history, medicines and how long symptoms last. Fever with diarrhea is not a diagnosis by itself.

Sip water or oral rehydration fluid often, and replace fluids after each loose stool as tolerated. Rest and eat small portions of simple food when hungry. Wash hands thoroughly, clean shared bathroom surfaces and avoid preparing food for others while unwell. Check your temperature and note blood, mucus, vomiting, abdominal pain and urine output. Do not use leftover antibiotics, because the cause may be viral or require a different treatment.

Arrange same-day medical advice for blood or black stool, severe abdominal pain, repeated vomiting, worsening fever, signs of dehydration or diarrhea that is not settling. Emergency help is needed for fainting, confusion, difficulty breathing or inability to keep fluids down. Babies, older adults, pregnant people and those with chronic illness should seek advice sooner because dehydration can develop rapidly.
