Diaper rash in folds of skin

Creases can remain damp after washing or after a wet nappy, and rubbing can damage the surfaces touching each other. Irritation may affect the groin, buttock creases, lower abdomen, or upper thighs. A smooth red patch can be irritant dermatitis. A shiny, intensely red rash involving the creases with small spots at the edge may suggest yeast, while crusting or oozing raises concern for another infection.

Change nappies promptly, rinse gently, and pat every crease dry without rubbing. Give air exposure when practical and avoid tight clothing or nappies that press into folds. Use a bland protective barrier if advised, keeping layers thin so moisture is not trapped. Stop fragranced wipes, bubble bath, and new products if they seem to sting. If the rash has a yeast-like pattern or fails to improve, ask a clinician whether topical antifungal treatment is appropriate rather than guessing.

Arrange medical review when the rash spreads beyond the folds, cracks, blisters, bleeds, oozes, or causes substantial pain. Seek prompt help for fever, worsening swelling, pus, or a child who seems unusually unwell. A recurrent rash may need assessment for the trigger and a tailored skin-care plan.
