Diaper rash at 2 weeks old: what parents should know

A newborn’s skin can become red and sore where the nappy traps dampness. A rash limited to areas covered by the nappy often improves with reducing wet contact. Bright red skin in the folds, small nearby spots, blisters, raw areas, or yellow crusting can point to a different process and should be described to a clinician.

Change wet or soiled nappies promptly. Rinse with lukewarm water or use a fragrance-free wipe, then pat rather than rub. Give short periods without a nappy when practical, and apply a simple protective barrier to clean, dry skin. Avoid fragranced products, scrubbing, talc, and medicated creams unless prescribed. Contact a clinician promptly for a two-week-old with fever, poor feeding, unusual sleepiness, fewer wet nappies, spreading redness, warmth, pus, open sores, blisters, or rapidly worsening pain. Seek urgent help if the baby appears seriously unwell or is difficult to wake. Newborns can become unwell quickly, so do not rely on the rash alone to judge severity.

Ask whether the pattern fits irritation, yeast, infection, or another skin problem; which cleanser and barrier are suitable; whether any cream is safe for this age; and how soon the skin should be reviewed if it does not improve.
