Diaper Heat Rash: What to Know

In a diaper area, warmth, humidity, tight clothing, and friction can prevent skin from cooling and drying. The rash may look red or prickly, with tiny bumps, and can make a child unsettled. It may also occur around the waist or in nearby covered skin. Skin redness, bumps, and a rash can have other causes, so the appearance does not confirm heat rash.

A rash that involves moist folds, has satellite spots, a ring-like edge, marked scaling, pus, or persistent soreness may need assessment for a different problem. Avoid assuming that every diaper rash is caused by heat.

Change diapers frequently, clean with lukewarm water or a gentle fragrance-free product, and pat rather than rub. Give the skin brief air exposure when practical, choose a well-fitting diaper, and avoid extra layers or fragranced products that trap heat.

A plain protective emollient may reduce friction when recommended for your child. Do not apply talcum powder, and do not use topical corticosteroids or other medicated creams without advice about the correct product and location. Keep the child comfortably cool rather than applying ice directly.

Contact a clinician if the rash is worsening, painful, blistered, weeping, crusted, associated with fever, or not settling after gentle cooling and hygiene. Seek urgent help if the child seems very unwell, has breathing difficulty, facial swelling, or a rapidly spreading rash.

Ask whether the rash fits heat rash or another condition, which products are suitable for the child’s age, and what changes would mean that the skin needs examination. Mention recent fever, medicines, diarrhoea, and any new diaper or cleanser.
