What should you know about DHI hair transplant procedure?

Short answer
A DHI hair transplant procedure involves assessing the donor area, collecting follicular units, and placing them into planned sites with an implanter pen under local anaesthesia.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Main stages
- Plan, collect, load, and implant follicular units.
- Anaesthesia
- The scalp is commonly numbed with local anaesthesia.
- Personalisation
- Hair pattern and donor supply shape the plan.
What happens during DHI
The clinician first reviews the cause and pattern of loss, designs the hairline, and marks the donor and recipient areas. Follicular units are then removed from the donor zone, commonly at the back or sides of the scalp. Each selected unit is loaded into an implanter pen and inserted at a planned angle, direction, and depth.
The procedure is usually performed with local anaesthesia, so you remain awake while the scalp is numbed. Sensations can include pressure, movement, or brief discomfort. DHI describes the placement method; it does not mean that every patient needs the same number of grafts or has the same recovery. [FUE Hair Transplant](/treatments/hair-restoration/fue-hair-transplant/) is a related restoration approach.
Prepare for the procedure
Provide a complete list of medicines, allergies, scalp conditions, and previous procedures. Clarify whether your hair loss is stable, whether the donor area can support the design, and how the clinician will protect existing hair. Ask who performs each stage, how grafts are stored, and what plan is used if native hair continues to thin.
Follow pre-procedure instructions about washing, medicines, alcohol, smoking, and activity. Wear comfortable clothing that does not need to be pulled over the scalp, and arrange transport if the clinic advises it because of medication or fatigue.
When to seek care
Before treatment, report an inflamed, painful, infected, or heavily scaling scalp so it can be assessed. After the procedure, contact the clinic for increasing pain, spreading redness, fever, pus, persistent bleeding, or sudden worsening swelling. Breathing difficulty or swelling affecting vision requires urgent medical help.
Questions for your doctor
Ask how the hairline will be designed, how donor follicles are selected, what anaesthesia is used, how long the session may take, what recovery restrictions apply, and how future hair loss will be managed.

Questions people ask
What does the implanter pen do?
It holds a follicular unit and helps the clinician place it into the recipient area at a selected angle and depth.
Is DHI the same as every FUE transplant?
No. FUE describes follicle extraction, while DHI commonly refers to a direct placement method using an implanter pen.
