A dermatofibroma is a firm nodule that may pull the overlying skin inward when pinched from the sides. This is often called a dimple sign. It is a useful clue, not a required feature. A shallow, deep, flat, or less typical nodule may not show it, and repeated pinching can irritate the skin.

A missing dimple does not identify another condition on its own. The clinician also considers colour, border, firmness, exact location, duration, symptoms, and any change over time.

Stop testing the spot repeatedly and take a photograph in consistent lighting instead. Note whether it is enlarging, becoming painful, changing colour, or catching on clothing. If the lump has not been assessed, book a skin examination; dermoscopy may add useful detail, while a biopsy or excision may be considered when the appearance remains uncertain.

Arrange prompt medical review for rapid growth, an irregular edge, marked colour variation, persistent pain, ulceration, or bleeding without an obvious scrape. These changes call for assessment because a dimple test cannot safely classify a changing lesion.

Ask whether the nodule’s overall features fit a dermatofibroma despite the absent dimple, whether dermoscopy is useful, and what follow-up interval is sensible. If removal is proposed, ask how the result will be confirmed and what scar to expect.
