Dermal fillers under the eyes: what to know

A hollow beneath the lower eyelid can create a shadow that looks like dark circles. Filler may add support in a selected plane and soften the transition between the lower eyelid and cheek. It is less likely to help when the main concern is loose skin, prominent fat pads, pigmentation, allergy-related puffiness, or surface wrinkles. Facial volume loss and wrinkles can coexist, so the clinician should identify what is creating the appearance before suggesting an injection.

Hyaluronic acid filler may be discussed for this area, but product behaviour, skin thickness, swelling tendency, and previous procedures matter. A careful history should include eye disease, allergies, medicines, prior filler, facial surgery, and recent dental or cosmetic treatment.

Ask for an examination in natural light and a clear explanation of the proposed entry points, depth, product, and alternatives. Do not choose treatment solely from photographs or a promise of a particular look. A qualified medical injector should explain how they would respond to vascular or eye-related complications.

Mild swelling, bruising, tenderness, and temporary asymmetry may follow. Use only the aftercare recommended for you, avoid rubbing the eyelid area, and allow the tissues to settle before judging the result. Alternatives can include skin treatments, Botox for selected movement-related lines, a collagen biostimulator in a different indication, or fat transfer when surgery is appropriate.

Call the treating clinic urgently for escalating pain, unusual skin pallor or darkening, marked swelling on one side, or a new visual symptom. Go to emergency care for sudden loss or blurring of vision, severe eye pain, weakness, confusion, or difficulty speaking after an injection. Prompt assessment matters for any new eye or neurological symptom.
