What should you know about depression chest pain?

Short answer
Depression and emotional distress can occur with chest tightness or pain, but chest pain should never be assumed to be caused by depression without medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Key point
- Chest pain requires assessment even when depression or anxiety is present.
- Emergency signs
- Breathlessness, fainting, spreading pain or coughing blood need immediate help.
- Record
- Note timing, location, triggers and associated symptoms.
Why chest pain needs attention
Anxiety or panic can tighten chest muscles, change breathing and create pressure, tingling or a racing heartbeat. Depression may coexist with anxiety, but these sensations also occur with conditions such as acid reflux, gastritis, angina, pneumonia or a peptic ulcer. A burning feeling after meals, pain linked to breathing, fever, cough or a rash may provide useful clues, but symptoms overlap.
The pattern, severity, triggers, associated symptoms and personal health history help a clinician decide what assessment is needed. Do not use a depression label to explain new, persistent or unexplained chest pain.
What to do with new symptoms
Stop strenuous activity, sit upright and note when the pain began, where it is, what it feels like, and whether it spreads. Arrange prompt medical assessment for new or recurring pain, especially if you have heart or lung risk factors. Follow prescribed treatment only; do not borrow someone else’s heartburn or anxiety medicine.
If panic seems to be contributing and no emergency signs are present, breathe slowly without forcing deep breaths and stay with a trusted person while arranging care. A clinician can assess both physical causes and panic disorder or depression.
When chest pain is an emergency
Call local emergency services immediately for pressure, squeezing or severe pain; pain spreading to the arm, jaw, back or shoulder; breathlessness, fainting, cold sweating, marked weakness, confusion, coughing blood, or sudden pain after a period of immobility. Do not drive yourself. These symptoms can signal serious heart or lung problems, including angina or pulmonary embolism.
Questions for your doctor
Ask which causes fit the timing and triggers, whether you need an examination or tests, how to recognise recurrence, and how emotional symptoms should be treated alongside physical symptoms. Tell the clinician about medicines, smoking, pregnancy, recent travel or immobility, and any family history of heart disease.
