Demodex folliculorum infestation: what to know

Demodex folliculorum are microscopic organisms associated with hair follicles, especially around the face and eyelashes. Many people have them without symptoms. Trouble can arise when the skin develops inflammation, leading to roughness, small bumps, redness, itch, burning or lash-line debris.

The word infestation can sound alarming, but it does not automatically mean poor hygiene or a dangerous infection. Facial redness and bumps can also reflect rosacea, acne, bacterial folliculitis, allergy or irritation. A clinician interprets the skin pattern rather than relying on the label alone.

Wash gently, avoid abrasive exfoliation and stop picking at follicles. Keep face cloths, pillowcases and makeup tools clean, and avoid sharing products that touch the face or eyes. Record where symptoms occur and whether heat, cosmetics or a new product makes them worse.

Book a dermatology review when symptoms continue, recur or involve the eyelids. A clinician may inspect the skin and choose a targeted treatment based on the findings. Do not apply insect products or non-prescribed chemicals to the skin, eyelashes or eyes.

Seek prompt care for rapidly spreading redness, increasing warmth or tenderness, pus, fever, or feeling unwell. Arrange urgent eye assessment for pain, light sensitivity, vision change or pronounced swelling around the eye. A widespread rash, facial swelling or a sore that does not heal also needs review.

Ask what finding supports Demodex involvement, which alternative diagnoses were considered, whether eyelid care is needed, which products to avoid, and what change would show that treatment is working.
