What should you know about the dark side of epidural steroid injection?

The injection places a corticosteroid, sometimes with a local anaesthetic, near the epidural space around spinal nerve roots. It is used for selected patterns of back or neck pain, including pain related to a herniated disc, sciatica, or spinal stenosis. Relief may be temporary, absent, or accompanied by short-lived soreness. Possible effects include headache, flushing, sleep or mood changes, a temporary rise in blood glucose, infection, bleeding, or nerve irritation. Your medical history affects the balance of benefit and risk.

Tell the clinician about blood-thinning medicines, infection, diabetes, allergies, pregnancy, and previous reactions to injections. Arrange transport if numbness or weakness is expected after the procedure. Follow the activity and wound-care instructions, note changes in pain or sensation, and use only the medicines advised for you. An injection is usually one part of a wider plan that may include movement rehabilitation and treatment of the underlying cause.

Contact the treating service urgently for fever, increasing redness or drainage, severe or persistent headache, new weakness, spreading numbness, or loss of bladder or bowel control. Emergency help is needed for difficulty breathing, collapse, or rapidly worsening neurological symptoms. New severe pain after the procedure should be assessed rather than assumed to be a normal reaction.

Ask what diagnosis the injection is targeting, what improvement would count as useful, and how long to wait before judging the result. Discuss alternatives, medicine adjustments, your personal risk factors, and the plan if symptoms return or do not improve.
