What should you know about cylinder cast for patella fracture?

A cylinder cast usually extends from the upper thigh to below the knee or toward the ankle, holding the leg in a straighter position. The exact design depends on the fracture pattern, swelling, skin condition, and whether the kneecap needs more stabilisation. Imaging and examination guide the decision.

Plaster of Paris or fibreglass cast material may be used. A cast supports the injured area but does not by itself show whether the bone is staying aligned. Follow-up imaging can help the orthopaedic team decide whether casting remains appropriate or whether another treatment is needed.

Keep the cast dry and never push objects inside it to scratch. Elevate the leg as advised, move the toes regularly if allowed, and use crutches only in the way demonstrated. Do not put weight through the leg unless the treating team has specifically permitted it.

Check the exposed skin and toes for changes, and attend every review. Once the cast is removed, stiffness and reduced thigh strength are common reasons for a gradual rehabilitation plan and physiotherapy.

Get urgent help for increasing pain or tightness, numbness, pale or blue toes, inability to move the toes, marked swelling, a wet or cracked cast, or a bad smell with drainage. Sudden breathlessness or chest pain also needs emergency care.

Ask when you may stand or bend the knee, how to bathe safely, when follow-up imaging is planned, and which symptoms mean the cast needs adjustment. Ask whether closed reduction, fracture fixation surgery, or later physiotherapy may be relevant to your fracture.
