What is happening

The iliotibial band is a strong strip of connective tissue running along the outer thigh. Repeated bending and straightening of the knee can irritate tissues near its outer side, causing a focused ache or sharp pain during running, downhill walking or repeated activity. Knee pain has several possible causes, so an examination considers the hip, knee, ankle, training pattern and footwear. Pain that occurs at rest, follows a major injury or comes with locking or giving way needs a broader assessment.

Building recovery

Temporarily reduce the distance, speed, hills or frequency that trigger pain, while keeping comfortable low-impact movement if possible. Physiotherapy can strengthen the hip and trunk, improve single-leg control and build a staged return to sport. Gait retraining may help when stride, cadence or landing habits add repeated load. Sports massage can be used as an adjunct for muscle tension, but it does not replace load management or strengthening. A cortisone injection is a clinician-guided option in selected cases, not a substitute for correcting the provoking pattern.

When to seek care

Arrange an assessment when outer-knee pain keeps returning, changes your stride, or prevents ordinary activity. Seek prompt care after a twisting injury, sudden swelling, inability to bear weight, fever, calf swelling, numbness or a knee that locks or gives way. Those signs may indicate a problem other than IT band irritation.
