PET scan and CT scan: how they work together

PET detects the distribution of a small amount of radioactive tracer, while CT maps anatomy using X-rays. When performed together, the images can help a clinician locate an area of altered tracer uptake more precisely. The finding still requires interpretation in the context of the reason for scanning and prior results.

CT depicts the size, shape and position of organs, lymph nodes, bones and other structures. PET reflects how the tracer is taken up in tissues. Increased or reduced uptake is not, by itself, a diagnosis: normal processes, inflammation and treatment effects can influence the pattern.

The CT portion may be used mainly to align the PET signal, or it may be planned as a fuller diagnostic CT. That distinction affects preparation, breathing instructions and whether an imaging contrast agent is given. The report should explain the significant PET and CT observations together.

Preparation depends on the tracer and the purpose of the scan. Use the appointment instructions for eating, drinking, physical activity and medicines rather than relying on general advice. Tell the nuclear medicine team about diabetes, pregnancy or possible pregnancy, breastfeeding, allergies, kidney problems and previous reactions to contrast.

Arrive with the requested records and allow the team to check your history before tracer administration. There is usually a quiet uptake period before imaging. During the scan, keep still and follow instructions about breathing. Ask the centre beforehand if pain, anxiety or difficulty lying flat may prevent you from staying in position.

Do not wait for a PET-CT appointment or result if you develop severe breathing difficulty, new one-sided weakness, sudden confusion, loss of consciousness, uncontrolled bleeding or intense chest pain. Seek emergency help because these symptoms need immediate clinical assessment independent of the imaging plan.

Tell staff at once if you become unwell during the visit. After an intravenous contrast agent, breathing trouble, facial or throat swelling, widespread hives or faintness require immediate attention. For new but non-emergency symptoms before the scan, contact the referring team so the imaging plan and timing can be reviewed.

Ask which tracer is planned, which body regions will be covered and whether the CT portion is for image alignment or detailed diagnosis. Confirm whether intravenous or oral contrast will be used, which preparation instructions apply to your medicines, when the report is expected, and who will explain how the results affect the next decision.
