Croup cough and sore throat: what to know

Croup is a swelling-related illness of the upper airway that can create a harsh, barking cough, hoarse voice, or noisy breathing. A sore throat may occur with the same viral illness, although throat irritation can also come from post-nasal drainage or reflux. A common cold, influenza, COVID-19, tonsillitis, or strep throat can present with throat pain; glandular fever is another possibility when marked tiredness and swollen neck glands accompany the illness. Acid reflux and allergic rhinitis may irritate the throat without causing a typical infection.

Keep the child or adult calm, because crying and agitation can make upper-airway noise more noticeable. Offer frequent fluids and soft foods if swallowing is uncomfortable. Rest, avoid smoke and strong fragrances, and use age-appropriate medicine only according to its label or a clinician’s advice. Do not use steam inhalation, and do not give antibiotics unless prescribed. Note whether the cough is worse at night, whether a whistling or harsh sound occurs while breathing in, and whether drinking and urination remain normal.

Seek emergency help for struggling to breathe, blue or grey lips, pauses in breathing, severe drowsiness, drooling with inability to swallow, or noisy breathing that continues while resting. Arrange prompt medical assessment if the cough is worsening, the person cannot drink, dehydration is developing, or symptoms are not settling. Infants and children with suspected croup deserve a lower threshold for assessment because airway swelling can change quickly.
