Crohn's disease in the terminal ileum

The terminal ileum is the final section of the small intestine, where it joins the large intestine. Crohn's disease in this area may cause cramping or pain in the lower right abdomen, loose stools, urgency, tiredness, reduced appetite, or weight loss. Inflammation can also produce swelling and narrowing that makes passage through the bowel more difficult.

The location of pain is not enough to identify the cause. Appendicitis, infection, gallbladder problems, and other conditions can also cause right-sided abdominal pain, so a new or changing symptom needs clinical assessment.

Arrange a GP Consultation if pain, diarrhoea, urgency, or tiredness continues or differs from your usual pattern. Describe stool frequency and appearance, appetite, weight change, fever, and whether pain follows meals. A Blood Test may help assess inflammation and anaemia; your clinician will decide whether further bowel evaluation is needed.

Keep hydrated and avoid restrictive diets unless advised. If eating is difficult, ask about practical ways to protect nutrition while the bowel is being assessed. Continue prescribed Crohn's treatment unless your healthcare team gives different instructions.

Get urgent medical help for increasing right-sided pain, repeated vomiting, a swollen abdomen, inability to pass stool or gas, heavy or black bleeding, fever with severe weakness, or faintness. These symptoms may indicate obstruction, infection, bleeding, or another urgent abdominal problem.
