Crohn's disease pain: causes and next steps

Pain can feel cramping, aching, or sharp and may occur before a bowel movement. Its position, duration, relation to meals, and change over time provide useful clues. Active inflammation may occur with diarrhoea, urgency, tiredness, or reduced appetite. A blockage can cause worsening cramps, swelling, vomiting, and difficulty passing stool or gas, which needs prompt assessment.

Pain intensity alone does not show how much inflammation is present. A clinician considers the full symptom pattern, examination, medicines, and test results.

Record when pain starts, where it is, what it feels like, bowel changes, vomiting, fever, and food or medicine triggers. Keep drinking unless a clinician has told you otherwise, and take prescribed medicines as directed. Arrange a GP consultation when pain is recurrent, persistent, or different from your usual pattern. A blood test and other investigations may help identify inflammation or another cause.

Seek urgent care for severe or rapidly worsening pain, a rigid or swollen abdomen, repeated vomiting, inability to pass stool or gas, faintness, fever with marked weakness, or blood in the stool. Do not wait for a routine appointment when these changes occur.
