Crohn's disease and erythema nodosum

The lumps are usually deeper than a surface rash and may look red or violet before fading. They can occur with joint aches, tiredness, fever or digestive symptoms. In a person with Crohn's disease, this skin reaction may reflect inflammatory activity, but the skin findings alone cannot show how active the bowel disease is.

Other infections, medicines and inflammatory conditions can also cause similar tender nodules. A clinician may examine the distribution and check the wider history before deciding whether the lumps fit erythema nodosum or another condition.

Rest the legs when they are sore and elevate them if that feels comfortable. Loose clothing and gentle skin care reduce rubbing. Do not massage, squeeze or puncture the lumps. Keep a record of new bowel symptoms, joint pain, fever and medicine changes to share with the gastroenterology or dermatology team.

Treatment focuses on the underlying trigger and symptom relief. Your clinician may adjust Crohn's disease management or discuss an anti-inflammatory treatment. Oral corticosteroid treatment can be considered in selected situations, but it needs medical supervision because suitability depends on infection risk and other health factors.

Arrange prompt assessment for new painful shin lumps, especially with fever, worsening abdominal pain, persistent diarrhoea, blood in the stool or swollen joints. Seek urgent care for severe leg swelling, chest pain, breathlessness, fainting, rapidly spreading redness or a very painful single-sided leg.

Ask whether the lumps are consistent with erythema nodosum, whether your Crohn's disease needs review and which pain-relief options are safe for you. Ask what symptoms should prompt urgent contact and whether a corticosteroid is appropriate in your circumstances.
