Crohn's disease and back pain

Crohn's disease can be associated with inflammation affecting the joints near the spine. This type of pain may feel worse after rest, cause morning stiffness, or improve gradually with movement. Other back pain may follow posture, lifting, an injury, or reduced activity during bowel symptoms. Abdominal pain can also be felt toward the back, which makes the pattern worth describing carefully.

Record where the pain is, when it started, whether stiffness lasts after waking, and whether it changes with movement or bowel symptoms. Mention leg weakness, numbness, fever, or urinary changes rather than assuming they are part of Crohn's disease.

Keep moving gently within your comfort, avoid prolonged bed rest, and use positions that support your back. Ask a clinician before taking anti-inflammatory pain medicines, because some may aggravate bowel symptoms or interact with treatment. Do not alter prescribed Crohn's medicines without advice.

A GP Consultation can assess the pain pattern and decide whether you need further care. A Blood Test may help assess inflammation or anaemia when the wider clinical picture calls for it, but it cannot identify every cause of back pain.

Get urgent help for new leg weakness, loss of bladder or bowel control, numbness around the groin, severe pain after an injury, fever with a very painful back, or severe abdominal pain with vomiting. Seek prompt review for persistent night pain, progressive stiffness, or back pain with worsening diarrhoea or marked fatigue.

Ask whether the pattern suggests spinal joint inflammation, a mechanical back problem, or pain referred from the abdomen. Ask which pain relief is suitable with Crohn's disease and whether you need physiotherapy, blood work, or specialist review.
