How cough fits in

Scarlet fever is linked to infection with group A streptococcal bacteria and commonly begins with a sore throat and fever. The rash may feel rough like sandpaper and can spread across the body. Swollen glands, headache, nausea, or a coated tongue may also occur. A cough is less typical than the throat symptoms, so cough with a runny nose or hoarseness may point towards a viral illness instead; symptoms can overlap, however.

Coughing does not rule scarlet fever in or out. A clinician may examine the throat, skin, and glands and decide whether testing or treatment is needed. Do not use the cough alone to decide whether antibiotics are appropriate.

Supportive care and spread reduction

Offer frequent fluids and rest, and use age-appropriate fever or pain relief as directed. Avoid sharing cups, towels, or utensils, and wash hands after coughing. Keep children home from school or childcare until a clinician confirms when it is appropriate to return. If prescribed antibiotics, take them exactly as directed and complete the course unless the prescriber changes it.

When assessment matters

Arrange prompt medical advice for fever with a sore throat and rough rash, especially when swallowing is painful or glands are enlarged. Seek urgent help for trouble breathing or swallowing, drooling, severe dehydration, unusual sleepiness, a rapidly spreading rash, or a child who looks seriously unwell. Return for review if symptoms worsen or do not begin to settle after treatment.
