What complications can happen?
Diphtheria bacteria can produce a toxin that damages tissues beyond the throat. A thick coating in the throat may obstruct breathing or make swallowing difficult. The toxin can also injure the heart, causing an abnormal heartbeat or reduced pumping strength, and may affect nerves involved in swallowing, voice or movement. Severe illness can lead to weakness and breathing difficulty when the muscles that support breathing are affected. A sore throat can be one presentation of diphtheria, but it does not establish the diagnosis on its own.
What should you do?
Diphtheria needs assessment in a medical setting. Treatment may include antibiotics to stop bacterial growth and an antitoxin to limit toxin activity, with observation of breathing, swallowing, heart rhythm and nerve function. Do not try to remove a throat coating yourself. Avoid close contact with others until a clinician confirms the infection is no longer transmissible. Household or close contacts may need testing, preventive treatment or vaccination advice. Follow the care team’s instructions about isolation and complete prescribed treatment.
When should you seek urgent care?
Seek emergency help for noisy or difficult breathing, bluish lips, inability to swallow saliva, rapidly increasing neck swelling, fainting, chest discomfort, a racing or irregular heartbeat, new severe weakness or confusion. These signs can reflect airway obstruction, heart involvement or toxin-related nerve injury. If diphtheria is suspected after a sore throat with a grey or white throat coating, arrange urgent medical assessment rather than waiting for symptoms to settle.
