Complications of Cervical Cerclage

Mild pelvic pressure, spotting, or cramping may occur after the stitch, especially soon after the procedure. More concerning problems include heavy bleeding, fever, unpleasant-smelling discharge, increasing pelvic pain, regular tightening, or leakage of fluid. The stitch can irritate or injure the cervix, and contractions may signal preterm labour. A cerclage does not prevent every cause of early birth, and it may need urgent review if labour begins or the membranes rupture.

The stitch is usually removed before birth, but timing depends on your pregnancy and the reason it was placed. If removal is delayed while labour is progressing, the cervix can be injured, so follow the maternity team's instructions.

Rest according to your clinician's advice and follow instructions about activity, sex, medicines, and follow-up. Keep the area clean without inserting anything into the vagina unless your maternity team says it is suitable. Note the colour and amount of any bleeding or fluid, whether pain comes in waves, and your temperature. Contact the team that placed the cerclage if symptoms are new or worsening. Seek urgent maternity assessment for heavy bleeding, severe or persistent abdominal pain, regular contractions, a gush or continuing trickle of fluid, fever, faintness, or discharge with a strong unpleasant smell. Reduced fetal movement later in pregnancy also needs urgent advice. Do not wait for a routine appointment if you think labour has started or your waters may have broken.

Ask why the cerclage was recommended, which symptoms are expected, when it should be removed, what activities to avoid, and which hospital to contact outside clinic hours. Confirm how follow-up will check the cervix and membranes.
