What should you know about complications of scarlet fever?

Short answer
Complications of scarlet fever can affect the ears, throat, joints, kidneys, or heart, so persistent fever or worsening illness needs medical review.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Cause
- Scarlet fever is linked to a streptococcal throat infection.
- Watch for
- Swallowing difficulty, reduced urine, joint pain, or breathlessness.
Possible complications
Scarlet fever is linked to a streptococcal throat infection. Most people recover with appropriate treatment, but the infection can extend to nearby areas, causing an ear infection, sinus symptoms, or a collection of pus near the tonsils. Less commonly, inflammation after the infection can affect the kidneys, joints, or heart.
A rash, sore throat, fever, and swollen glands can occur with scarlet fever, but these symptoms do not predict which complication will develop. New swelling, dark urine, reduced urine, painful joints, chest discomfort, or breathlessness should be reported rather than dismissed as part of recovery.
Steps during recovery
Use prescribed antibiotic treatment exactly as directed and complete the course unless the prescriber changes it. Rest, drink suitable fluids, and use recommended pain or fever relief according to the label or clinical advice. Avoid sharing cups, towels, or utensils while infection-control advice applies, and wash hands regularly.
Arrange follow-up if the throat pain, fever, rash, or swollen glands are not settling, or if symptoms return after improvement. Tell the clinician about reduced urination, new joint pain, or a new medicine reaction.
When to get urgent help
Seek urgent care for difficulty breathing or swallowing, drooling, severe one-sided throat pain, inability to drink, confusion, fainting, or rapidly worsening swelling. Prompt medical review is also needed for blood or cola-coloured urine, marked swelling, very little urine, chest pain, or new breathlessness after scarlet fever.
Questions for the clinician
Ask whether the throat infection needs testing, how long the person should stay away from close contacts, which symptoms suggest a kidney or joint problem, and whether follow-up urine or blood checks are needed. Confirm the correct antibiotic schedule and what to do if a dose is missed.

Questions people ask
Can scarlet fever affect the kidneys?
An inflammatory kidney complication can follow the infection and may cause swelling, dark urine, or reduced urination.
Does a persistent sore throat always mean a complication?
No. It can have several causes, but ongoing or worsening symptoms should be assessed for infection or a local complication.
