Common sites for prostate cancer metastasis

Prostate cancer most often spreads to nearby lymph nodes and bones, but it can also reach organs such as the liver or lungs. Bone spread commonly involves the spine, pelvis or ribs. The pattern differs from person to person, and a scan is needed to identify where disease is present.

Metastasis means cancer cells have travelled beyond the prostate and formed deposits elsewhere. Bone deposits may cause a persistent, focused ache or weaken a bone. Enlarged lymph nodes may cause swelling, while liver or lung involvement can affect appetite, energy or breathing. Some deposits cause no symptoms and are first seen on imaging. The location and extent of spread help the cancer team choose treatment and symptom support.

Report new, persistent pain, weakness, unexplained swelling, breathlessness, appetite change or worsening urinary difficulty to your cancer team. They may review your examination, blood tests and previous imaging, then arrange imaging suited to the concern. Keep a note of when a symptom began, exactly where it occurs, what makes it worse and whether it limits walking, sleep or daily activity. Treatment may focus on controlling the cancer, protecting bones and relieving symptoms; the choice depends on previous treatment and where disease is found.

Get urgent medical help for new leg weakness or numbness, difficulty walking, loss of bladder or bowel control, or severe back pain that is new or rapidly worsening. These changes can occur when disease affects the spine and may need immediate assessment. Seek emergency care for sudden severe breathlessness, chest pain, confusion, or a suspected broken bone. Do not wait for a routine oncology appointment when these red flags appear.

Ask which areas are involved, what each scan shows, and whether any bone or spinal site needs protection. You can also ask what the treatment is intended to achieve, how response will be checked, which side effects deserve a same-day call, and whom to contact outside clinic hours. If pain or mobility is changing, discuss pain relief, physical support and whether bone-strengthening treatment is suitable.
