Cold feet, hot body and fever: what to know

Fever can cause chills, shivering, sweating, warm skin on the trunk, and cooler hands or feet. The sensation alone does not identify the infection. Viral illnesses such as the common cold, COVID-19, bronchiolitis, or croup can cause fever, while bacterial infections such as cellulitis may also be possible. Brucellosis matters if there has been relevant animal or unpasteurised dairy exposure. Look at the whole pattern: breathing, alertness, hydration, pain, rash, and how quickly symptoms are changing.

Check the temperature with a reliable thermometer and record the reading, time, and any medicine taken. Offer frequent fluids and rest. Use light, comfortable clothing and bedding; avoid ice baths, very cold compresses, or heavy wrapping that can worsen shivering. Warm the feet gently with socks, but do not use direct high heat on numb skin. Follow the label or a clinician's advice for fever medicine, considering age, pregnancy, allergies, and other medicines.

Seek urgent medical help for trouble breathing, blue or grey lips, confusion, unusual drowsiness, a seizure, severe dehydration, a rapidly spreading rash, or skin that is unusually pale, mottled, painful, or numb. Get prompt advice if the person is a young infant, has a weakened immune system, is pregnant, or the fever is worsening or not settling. Cold feet with a hot body are not, by themselves, a measure of how serious the illness is.

Ask which symptoms point toward a respiratory infection, whether testing is useful, how much fluid is appropriate, and which fever medicine is suitable for the person's age and health. Mention travel, animal contact, new medicines, a rash, breathing changes, and the timing of cold feet.
