Check the temperature with a reliable thermometer, observe breathing and alertness, and consider the person's age, other symptoms, and medical conditions.

Why feet may feel cold

During a fever, the body may narrow blood vessels near the skin while it raises or resets its internal temperature. This can leave hands and feet cool while the trunk feels hot, followed by shivering or sweating. Viral illnesses such as a common cold, COVID-19, acute bronchitis, or croup may cause fever and chills. Bronchiolitis mainly affects young children and can make breathing harder. A hot, painful, spreading area of skin may suggest cellulitis. Brucellosis and chickenpox have different patterns and require a clinician to interpret the full picture.

What to do at home

Offer frequent fluids if the person can drink, use light clothing and bedding, and let them rest. Warm cool feet with socks or a blanket rather than intense heat. Follow the label or a clinician's instructions for fever medicine, taking account of age, pregnancy, allergies, and other medicines. Do not give aspirin to a child or teenager unless a clinician specifically directs it. Keep a simple record of temperature, fluid intake, urine, breathing, rash, and alertness.

When to seek care

Seek emergency help for difficult or fast breathing, blue or grey lips, a seizure, severe drowsiness, confusion, collapse, or a child who is hard to wake. Get urgent advice for a purple or rapidly spreading rash, severe dehydration, persistent vomiting, or a very unwell appearance. Babies, older adults, pregnant people, and people with weakened immunity should receive medical advice earlier.
