Rhabdomyolysis is the breakdown of muscle fibres, which can release myoglobin into the bloodstream and urine. Myoglobin can harm the kidneys, so waiting for the colour to clear is unsafe.

Why urine can turn dark

Myoglobin is an oxygen-binding protein inside muscle. When substantial muscle damage releases it, the kidneys filter some of it into urine, which may look tea-, cola- or reddish-brown. Rhabdomyolysis often presents with marked muscle pain, swelling or weakness, but the full combination is not always present.

Dark urine has other possible explanations, including blood in the urine, dehydration, liver or bile problems, foods and medicines. Its appearance alone cannot identify the cause. Recent extreme exercise, prolonged pressure on muscles, heat illness, trauma, seizures, infection or certain medicines can make muscle breakdown more concerning.

Stop exercising and seek same-day urgent medical assessment. Tell the clinician about recent activity, heat exposure, injuries, illnesses, supplements, alcohol or drug use, and all medicines. Blood and urine tests can check for muscle breakdown, kidney stress and disturbances in body salts; treatment may include monitored fluids and correction of complications.

Do not try to manage suspected rhabdomyolysis only by drinking large amounts of water. Fluid needs vary, and kidney or electrolyte problems require clinical monitoring. Avoid restarting strenuous activity until the cause is understood, symptoms have settled and a clinician has advised how to return safely.

When emergency care is needed

Go to an emergency department now for cola-coloured urine with severe muscle pain or swelling, pronounced weakness, very little urine, fainting, confusion, vomiting, breathing difficulty, chest symptoms or an irregular heartbeat. Urgent assessment is also warranted after crush injury, heat illness or prolonged immobilisation. These red flags can accompany kidney injury, dangerous electrolyte changes or extensive muscle damage.
