What should you know about cluster headache watery eyes?

During a cluster headache attack, intense pain around the eye can switch on automatic nerve responses in the face. The eye may tear, look red, or feel swollen; the nostril on that side may also become blocked or run. Many people feel unable to keep still. These signs can help a clinician distinguish the pattern from other headaches, but watery eyes alone have many possible causes, including irritation, allergy and eye disease.

The affected eye should be examined when symptoms are new, persistent between attacks, or accompanied by visual change. A description of which side waters and whether it happens only during pain can make the consultation more useful.

Record the side of the pain and tearing, how quickly both begin, how long they last, and whether the eyelid droops or the nose changes. Note contact-lens use, allergies and any eye products. Arrange a GP assessment for repeated attacks so other causes can be considered and a suitable treatment plan discussed. Do not put leftover antibiotic or steroid eye drops in the eye without advice.

Seek urgent care for severe eye pain with reduced vision, a fixed or unusual pupil, marked swelling, injury, or headache with weakness, confusion, fever and neck stiffness. Emergency help is also needed for a sudden explosive headache. New tearing that continues outside headache attacks deserves prompt review rather than being assumed to be cluster headache.
