Cluster headache on top of the head

Cluster headache commonly begins around one eye or the temple and may radiate upward over the forehead or scalp. The attack pattern matters as much as the map: severe one-sided pain may occur with tearing, eye redness, eyelid changes or a blocked nostril on the painful side. Restlessness is also common during an attack.

Pain focused on the crown can have many explanations, including tension-type headache, migraine, scalp tenderness, neck-related pain or other medical conditions. A clinician uses the complete history and examination rather than location alone.

During an episode, note whether the pain started near the eye and moved upward, or began on the crown. Record the side, associated eye and nose symptoms, time of onset, duration and anything that preceded it. Avoid alcohol if it has triggered attacks. Book a GP consultation for a new or recurring severe pattern so appropriate treatment can be considered.

Call emergency services for a sudden thunderclap headache, new weakness, trouble speaking, confusion, fainting, seizure, fever with a stiff neck, or headache after a serious injury. Seek prompt assessment for new vision changes, severe eye pain, or a rapidly changing pattern. Do not self-diagnose a crown headache as cluster headache.
