The short answer

The word chronic can be misleading because torsion is not considered a harmless long-term condition. An ovary may twist and partly untwist, producing pain that comes and goes before a more sustained episode.

How intermittent twisting may present

Ovarian torsion commonly presents with sudden pain on one side of the lower abdomen or pelvis, often with nausea or vomiting. Intermittent torsion can present with repeated episodes that settle, but improvement does not confirm that the ovary is safe or that blood flow is normal.

An enlarged ovary, cyst or mass can make twisting easier, although torsion can occur without a known cyst. Assessment may include an abdominal and pelvic examination, pregnancy testing and ultrasound. Scan findings can support the diagnosis, but clinicians interpret them alongside symptoms because no single finding settles every case.

What to do about recurring episodes

Do not wait for a routine appointment during a severe episode. Go for emergency assessment and describe where the pain started, whether it came on suddenly, whether similar attacks occurred before, and whether vomiting, bleeding or faintness is present. Avoid eating or drinking until the urgent-care team advises you, because an operation may be considered.

Treatment usually involves keyhole surgery to untwist the ovary and assess its condition. If a cyst is contributing, cyst removal may be discussed. The surgical approach depends on the findings, ovarian tissue and your circumstances, including fertility goals.

When emergency care is needed

Seek emergency care now for sudden or intense one-sided pelvic pain, pain with repeated vomiting, collapse, marked weakness or rapidly worsening symptoms. Urgency is based on these warning features and the risk to ovarian blood flow, even if an earlier pain episode stopped by itself.
