The short answer
The label is used for an imaging and clinical pattern rather than a conclusion that explains every case. Infection, non-infectious inflammation and other bone disorders can resemble one another, so treatment depends on establishing the most likely cause and exact site.
How clinicians clarify the diagnosis
Symptoms may include focused bone pain, tenderness or swelling that persists or returns. Some lesions are discovered during imaging for another concern. Sclerosis means the bone looks denser in that area; it does not by itself identify whether active infection is present.
Assessment brings together the symptom history, examination and imaging appearance. Blood tests may look for signs of inflammation, while specialist review may consider whether a tissue or bone sample is needed to distinguish infection from inflammatory, healing or tumour-like changes. Previous surgery, wounds, dental disease and antibiotic use can alter the interpretation.
Treatment and follow-up
Follow the plan from the orthopedic, infectious-disease, maxillofacial or other relevant specialist. If infection is confirmed or strongly suspected, treatment may include targeted antimicrobial medicine and, in selected cases, surgery to remove unhealthy tissue. Non-infectious disease follows a different pathway.
Protect the painful area from repeated heavy load until the team defines safe activity. Keep follow-up appointments even if discomfort settles, because symptoms and imaging changes do not always improve in parallel. Bone grafting may be considered when surgery leaves a defect or the bone needs structural reconstruction, but it is not required for every lesion.
When to seek care
Seek prompt medical care for increasing pain, redness, warmth, swelling, wound drainage, fever or chills. New difficulty using the affected limb, spreading numbness or rapidly worsening swelling also requires timely assessment.
Contact the treating team if medicine causes a rash, breathing trouble, severe diarrhoea or persistent vomiting; breathing trouble is an emergency. Report symptoms that return after treatment rather than restarting leftover antibiotics, which may be unsuitable and can interfere with later testing.
