What should you know about cholelithiasis clinical manifestations?

Cholelithiasis means stones in the gallbladder or biliary system. Many stones cause no noticeable symptoms. When a stone temporarily blocks the gallbladder outlet, a person may develop an episode of steady pain in the upper right or upper middle abdomen, often after eating. The pain can spread to the back or right shoulder and may occur with nausea, vomiting, bloating, or indigestion. Persistent blockage can inflame the gallbladder. A stone that moves into a shared bile duct may cause jaundice or dark urine, while obstruction near the pancreas can trigger pancreatitis. Obesity is one factor associated with gallstone formation, but symptoms do not establish a diagnosis.

Arrange a GP Consultation if episodes recur, interfere with meals or sleep, or leave you with ongoing tenderness. Record the start time, location, duration, food triggers, fever, bowel changes, and any yellowing of your skin or eyes. A clinician may examine you and arrange an ultrasound or blood tests. Avoid extreme dieting and seek tailored advice before making major changes to eating patterns. If a scan finds stones without symptoms, monitoring may be appropriate; symptomatic or complicated disease may need specialist treatment.

Seek urgent care for severe or persistent abdominal pain, fever, repeated vomiting, yellow skin or eyes, confusion, fainting, or inability to keep fluids down. Sudden intense pain that extends to the back also needs urgent assessment because it can occur with pancreatitis. These warning signs are based on the severity and pattern of symptoms, not on assuming that every abdominal symptom is caused by gallstones.
