What should you know about childhood rash with fever?

A rash may be flat, raised, hive-like, bruised-looking, scaly, or blistering. Fever can occur with viral illnesses such as Chickenpox, with a medicine-related Drug Rash, or with bacterial skin infection such as Cellulitis. A rash that changes quickly, involves the mouth or eyes, or appears with marked tenderness needs more attention than a mild, stable rash. The rash itself does not identify one diagnosis; clinicians use its distribution, speed of spread, recent medicines, contacts, and the child's behaviour and hydration.

Measure and record the temperature if you can, note when the rash began, and take clear photos in good light if it changes. Offer fluids and keep clothing comfortable. Do not start leftover antibiotics, creams, or new medicines without advice, because treatment can depend on the cause. Write down every medicine or supplement taken recently, including the first dose and any change after it. Contact a clinician promptly for an assessment when the child is worsening, the rash is spreading, or the cause is unclear.

Get emergency help if the child has trouble breathing, swelling of the lips or tongue, collapse, confusion, a seizure, a stiff neck, severe headache, or is difficult to wake. Urgent assessment is also needed for purple or bruise-like spots that do not fade with pressure, severe pain, rapidly spreading redness, skin peeling, dehydration, or a very unwell appearance. A newborn or a child with reduced immunity needs a lower threshold for medical review. Call ahead before attending if an infectious rash is possible.

Ask what the rash most resembles, whether any medicine should be stopped, which symptoms require emergency action, how to support fluids and comfort, and when follow-up is needed.
