Chest discomfort without pain: the key point

People may describe discomfort as pressure, heaviness, tightness, burning, fullness, tingling or an awareness that something feels different. The absence of pain does not by itself show that the cause is minor.

What chest discomfort can mean

Heart-related discomfort may occur with exertion or emotional stress and may spread to the arm, shoulder, back, neck or jaw. Angina is one possible cause; heart symptoms can also include breathlessness, sweating, nausea, unusual tiredness or light-headedness.

Acid reflux, gastritis or a peptic ulcer can cause burning or upper-abdominal discomfort that is felt behind the breastbone. A chest-wall muscle strain may worsen when you move, twist or press the area. Panic disorder can cause chest tightness with a racing heart, trembling or a feeling of fear. Lung conditions such as pneumonia or pulmonary embolism may cause discomfort with breathing, while shingles can cause sensitivity or burning before a rash appears.

What to do now

Stop strenuous activity and notice when the sensation started, what it feels like, how long it lasts and whether movement, meals or breathing affect it. Record accompanying symptoms and any recent illness, injury or new medicine.

Arrange medical advice for new, recurring or unexplained discomfort, especially if you have heart or lung disease, diabetes, high blood pressure, or a history of smoking. Do not use someone else’s medicine or assume that an antacid, pain reliever or calming technique has identified the cause.

When to seek urgent care

Seek emergency help immediately for chest discomfort that is severe, persistent, worsening or triggered by exertion, or that occurs with trouble breathing, fainting, cold sweat, nausea, marked weakness, confusion, or discomfort spreading to the arm, back, neck or jaw.

Urgent assessment is also needed for sudden breathlessness, coughing blood, a very fast or irregular heartbeat, or one-sided leg swelling. While waiting for help, rest, avoid driving yourself and follow the emergency operator’s instructions.
