Spotting and a chemical pregnancy: what to know

A chemical pregnancy is an early pregnancy loss detected by a positive pregnancy test before a pregnancy can be seen on ultrasound. Bleeding may start as light spotting and become more like a period, or it may remain light.

Light blood noticed on underwear or toilet tissue can occur in an ongoing early pregnancy as well as during a loss. Cervical irritation and other gynaecological causes can also produce spotting, so appearance alone does not identify the cause.

If a test was positive, repeat blood tests can show whether pregnancy hormone is rising or falling. An ultrasound may be considered according to symptoms, test results, and estimated pregnancy timing.

Use a pad or pantyliner if needed so you can observe the flow, and note whether it is increasing, contains clots, or is accompanied by pain. Avoid using colour alone to judge severity. Contact a maternity service or gynaecology clinician for advice after a positive pregnancy test, particularly when spotting persists or the pregnancy outcome has not been confirmed.

Keep any planned blood-test or scan appointments even if the spotting stops. The follow-up is intended to confirm that hormone levels and symptoms are resolving appropriately.

Seek urgent medical help if bleeding becomes heavy, pain is severe or mainly on one side, or you develop shoulder-tip pain, faintness, marked dizziness, weakness, or collapse. These signs require prompt assessment for significant blood loss or an ectopic pregnancy, regardless of whether earlier bleeding seemed minor.

Fever, worsening pelvic pain, or unpleasant-smelling discharge also warrants medical review because infection may need treatment.

You might ask whether your pregnancy hormone should be checked again, whether a scan is needed, what change in bleeding should prompt a return, and how the pregnancy outcome will be confirmed. Mention cervical screening history, bleeding after sex, unusual discharge, and infection risk if any apply, since they can guide examination and testing.
