Understanding the two modes

TENS sends mild electrical pulses through skin electrodes and is used to change how pain signals are perceived. NMES is set to produce a visible muscle contraction and may support muscle re-education or strengthening during rehabilitation. The modes are not interchangeable: electrode placement, intensity, pulse pattern, and session plan should match the intended goal. A strong sensation is not evidence that treatment will work better.

Using the device safely

Use the programme, electrode positions, and session schedule supplied by your physiotherapist or prescribing clinician. Apply intact electrodes to clean, dry skin, connect the leads before raising intensity, and increase stimulation gradually. TENS should feel firm but comfortable; NMES may create a controlled contraction without sharp pain. Switch the unit off before moving electrodes. Check the skin afterward and replace worn or poorly adhering pads.

Do not place electrodes across the chest, over the front or side of the neck, on the head, or over broken, infected, or numb skin. Get clinical guidance before use if you have a pacemaker or another implanted electronic device, are pregnant, have epilepsy, impaired sensation, circulation problems, or an unexplained painful area. Do not use stimulation while driving, bathing, or operating machinery.

When to stop and seek care

Stop stimulation and remove the electrodes if you develop burning pain, marked skin irritation, dizziness, palpitations, new weakness, or worsening symptoms. Seek urgent medical care for chest pain, breathing difficulty, fainting, sudden one-sided weakness, or a severe allergic reaction. For persistent redness, blistering, or pain after a session, keep the device off and contact the treating clinician before using it again.

Should TENS make the muscle contract?

A comfortable tingling sensation is commonly intended during TENS, while a visible contraction is generally the goal of NMES. Follow the mode-specific settings provided by your clinician.

Can electrodes be placed anywhere that hurts?

No. Placement affects the electrical path and may be unsafe over certain areas. Use the placement shown for your treatment and avoid the chest, neck, head, damaged skin, and areas with poor sensation unless a qualified clinician has directed otherwise.

What if the stimulation suddenly feels sharp?

Turn the unit off, inspect the leads, pads, and skin, and do not resume until the cause is clear. A dried, folded, loose, or damaged electrode can concentrate the sensation.
