The contraction may start during activity, at rest, or during sleep. The muscle can remain tender after it releases. Although many isolated cramps settle with gentle movement, frequent attacks or cramps accompanied by swelling, weakness, or other new symptoms should be assessed.

Why leg cramps happen

A cramp occurs when muscle fibres contract and do not relax normally for a time. Muscle fatigue, unfamiliar loading, sustained positioning, heat, and fluid loss can contribute. Pregnancy, some medicines, and conditions affecting nerves, circulation, metabolism, or kidney function may also be relevant in some people.

The word charley horse can also mean a muscle contusion after a collision. That injury has a different pattern: pain follows impact and may be joined by bruising or swelling rather than a visible knot that releases.

How to release a leg cramp

Stop the activity, steady yourself, and gently lengthen the affected muscle. For a calf cramp, straighten the knee as comfortable and bring the toes toward the shin; for the back of the thigh, slowly extend the knee. Calm breathing and easy walking can help once the spasm eases.

Afterward, avoid immediately repeating the movement that triggered it. Rehydrate after heat or sweating and return to activity progressively. If cramps follow a training change, review recovery, footwear, warm-up, and workload rather than relying on forceful stretching during pain.

When a leg cramp needs care

Seek urgent help for leg symptoms accompanied by chest pain, shortness of breath, fainting, or coughing blood. A leg that becomes markedly swollen, warm, red or otherwise discoloured without a clear minor injury also needs prompt evaluation rather than stretching or massage.

Consult a clinician if cramps are recurring, severe, disrupting sleep, linked to a new medicine, or accompanied by persistent weakness, numbness, skin changes, or difficulty walking. Do not stop a prescribed medicine on your own; ask whether the dose or an alternative should be considered.
