What the sensation can mean

A cramp is an involuntary tightening that can make a calf, thigh, or foot feel hard and intensely painful. It may happen at rest, during a stretch, or at night. Pregnancy changes posture, activity, circulation, and the load on muscles, so a cramp can occur without a direct injury. A muscle contusion, sometimes called a charley horse, is different: it follows a blow or strain and may cause tenderness, bruising, or pain with movement.

The location and pattern matter. Calf pain or thigh pain that is clearly linked to a brief cramp and settles is different from persistent one-sided pain, warmth, swelling, or tenderness. A clinician can distinguish a muscle problem from other causes using an examination and, when needed, further assessment.

What you can do

When a cramp starts, stop the activity, breathe steadily, and gently lengthen the affected muscle without bouncing. For a calf, keep the knee comfortably straight and draw the toes toward you only as far as feels tolerable. Gentle walking after the spasm eases may help, and supportive footwear, regular position changes, and fluids according to your maternity clinician’s advice can reduce strain.

For a bruise or strain, protect the area from further impact and ask your maternity team which cold or warm option is suitable. Do not take a medicine, use a supplement, or arrange vigorous massage without checking that it is appropriate in your pregnancy. Physiotherapy can help when cramps or movement limitations keep recurring.

When to seek care

Contact your maternity clinician promptly if one leg becomes swollen, warm, red, or persistently tender, or if pain continues despite rest. Seek emergency help for sudden breathlessness, chest pain, coughing blood, fainting, or severe weakness. These symptoms can signal a circulation or other urgent problem and should not be treated as an ordinary cramp. Also arrange review for repeated episodes, marked bruising without a clear injury, or difficulty walking.
