Cellulitis of the Perineum: What to Know

The perineum is the skin between the genitals and anus. Cellulitis there may cause redness, warmth, swelling, tenderness, or pain, sometimes after shaving, friction, a cyst, a wound, or a skin crack. Drainage, a foul smell, severe tenderness, blisters, or skin that becomes dusky or numb can suggest deeper or more aggressive infection. The appearance may change quickly, and an intimate examination is often needed to find the source and judge depth.

Contact a healthcare professional urgently for suspected perineal cellulitis. Keep the area gently clean and dry, wear loose clothing, and avoid shaving, squeezing, sexual activity, or unprescribed creams until assessed. If antibiotics are prescribed, follow the full instructions and return if symptoms do not improve as expected. Tell the clinician about diabetes, immune problems, recent procedures, wounds, or urinary and bowel symptoms.

Go to emergency care for rapidly spreading redness or swelling, severe or escalating pain, skin blisters, purple or black areas, numbness, fever with chills, vomiting, faintness, confusion, or difficulty passing urine. Pain that seems much greater than the visible skin change is especially concerning. A serious perineal infection can progress to Sepsis, so do not wait for a routine appointment when the area is worsening or you feel systemically ill.

Ask what caused the skin break, whether the infection is limited to the skin, and how the area should be cleaned or dressed. Confirm which changes mean immediate return, whether a surgical review is needed, and how diabetes or another health condition may affect treatment.
