Causes of spotting in the first trimester

Possible explanations include bleeding around implantation, a sensitive cervix after sex or examination, or an infection. Bleeding may also accompany an early pregnancy loss or an ectopic pregnancy. The amount and colour cannot confirm which explanation applies.

Clinicians interpret spotting alongside the pregnancy dates, location of discomfort, bleeding pattern and previous scan findings. Pink or brown marks may remain light and stop, while red bleeding may become heavier; none of these patterns alone gives a diagnosis. Cervical irritation tends to follow contact, whereas infection may bring unusual discharge, odour, itching or urinary discomfort.

An ectopic pregnancy develops outside the womb and can present with vaginal bleeding and pain, sometimes focused on one side. Early pregnancy loss can present with bleeding and cramps. Because symptoms overlap, clinical assessment is more reliable than trying to identify the cause at home.

Call your obstetrician, early-pregnancy service or maternity unit and explain how far along you are. Track the start time, colour and flow, and note cramps, one-sided pain, shoulder discomfort, faintness, fever or unusual discharge. A sanitary pad can help you observe changes; avoid inserting a tampon.

Depending on your symptoms and pregnancy stage, assessment may involve an ultrasound, blood tests taken at separate visits, urine testing, an examination or vaginal swabs. Bring details of medicines, your blood group if known, and previous ectopic pregnancy or miscarriage.

Seek urgent medical help if spotting occurs with severe abdominal or pelvic pain, pain mainly on one side, shoulder-tip pain, fainting, pronounced dizziness, heavy bleeding, fever, or passing clots or tissue. Rapidly worsening symptoms require emergency care because internal bleeding or another early-pregnancy complication may need immediate treatment.

Call local emergency services if you collapse, feel too faint to stand, have intense pain or are soaking a pad soon after putting it on. Do not drive yourself when you feel faint or acutely unwell.

Useful questions include whether the pregnancy location has been confirmed, whether repeat blood tests or another scan are needed, and what bleeding pattern should trigger a return. Ask whom to contact out of hours and whether you should temporarily change sex, exercise, travel or work plans.
