Why spotting can happen after a period ends

Spotting after a period ends may reflect remaining menstrual blood, hormone fluctuations, contraception, infection, pregnancy, or a cervical or uterine condition. The appearance can range from brown staining to fresh red blood. A one-off episode may settle, while bleeding that repeats or comes with other symptoms needs clinical assessment.

Brown spotting close to the end of a period can be residual blood exposed to oxygen before leaving the vagina. Cycle-related hormonal variation may cause light bleeding at another point. Hormonal pills, injections, implants or intrauterine contraception may change bleeding patterns, especially after a missed dose or recent change.

Pain during sex, pelvic discomfort or altered discharge may point toward irritation or an infection such as chlamydia. Endometrial polyps and uterine fibroids may present with bleeding between periods or heavier menstrual bleeding. Cervical dysplasia, cervical cancer and womb cancer may also present with abnormal bleeding; only an assessment can determine what is responsible.

Use a calendar or phone note to capture the first and last day of normal flow, each spotting episode, colour and approximate amount. Include bleeding after sex, cramps, pelvic pressure, discharge, contraception changes, medicines and pregnancy possibility. This record helps a clinician see whether bleeding relates to the cycle or another trigger.

If pregnancy could be possible, use a home test as directed. Book an appointment when the bleeding recurs, continues, follows sex, or is accompanied by pelvic symptoms. Depending on the history, evaluation may include a pregnancy test, infection swabs, examination of the cervix, blood tests or pelvic imaging.

Get urgent help if bleeding becomes heavy or occurs with collapse, faintness, severe abdominal or pelvic pain, shoulder-tip pain, or a positive pregnancy test. Prompt care is also appropriate for fever alongside pelvic pain or unpleasant-smelling discharge. Bleeding that occurs after menopause requires medical investigation, regardless of the amount.
